Why Case Management?

Case Management is a collaborative client-driven process for the provision of quality health and support services through the effective and efficient use of resources. Case Management supports the clients' achievement of safe, realistic and reasonable goals within a complex health, social, and fiscal environment.  ~Canadian Standards of Practice in Case Management 2009

The National Case Management Network (NCMN) has emerged as Canada's only membership association for Case Managers: the dedicated advocates helping patients and clients achieve better health-related outcomes.  Founded in 2006, the NCMN is a growing non-profit organization based in Toronto dedicated to the professional development of care workers through education, networking opportunities, and the establishment of standards of practice.

Throughout the nation, some Canadians with specialized, complex, and chronic health care experience limited access to, and gaps in health care. Each step of care involves various individual providers and transfers between settings representing a wide array of dedicated health care providers striving to meet care coordination issues. While these health care providers may or may not carry the title of Case Manager, they are nonetheless practicing Case Management. The effectiveness of NCMN ultimately rests with the commitment of its membership.

The NCMN serves the needs of those who work in Case Management whether they are working directly with patients/clients as a care manager, or they are supporting care management as a manager, leader, policymaker, or researcher in any of these sectors or organizations: Academics, Acute Care, Home and Community Care, Community Support Agencies, Department of National Defense, Social Services, Disability Management, Entrepreneurs, Insurance Agencies, Long Term Care, Mental Health and Addictions, Regional Health Authorities, Veterans Affairs Canada, Workplace and Safety Boards, and Rehabilitation.

A Cohesive Voice is a Strong Voice

Case Management is practiced by an ever increasing number of individuals and organizations. Case Managers are known for their skills as effective communicators, problem solvers, and visionaries. And for this reason benefit from joining voices with thousands of others who are speaking out and sharing experiences.

Practice domains cross disciplines and sectors locally, provincially, nationally and internationally. The benefits of Case Management increase exponentially when Case Managers integrate their work across these spectrums. National collaboration provides an opportunity to team up across the country and across sectors in applying evidence and best practice to care management.

The National Case Management Network of Canada developed the Canadian Standards of Practice for Case Management in 2009. The Standards represent a consensus of agreement based on the collective wisdom of the NCMN Standards Roundtable, NCMN Standards Workgroup, NCMN membership, and Case Management communities of practice.
